The burden of disease and death has remained disproportionate in developing countries despite increasing investments in health during the period of the Millennium Development Goals (MDGs). Between 1990 and 2014, more than $400 billion in health development assistance were invested in low income countries which saw a five-fold increase in annual allocations from about $6 billion in 1990 to about $36 billion in 2014. Besides, for every $1 in health development assistance, there was $20 of domestic resources invested in health.
Despite these significant resources, most low-income countries did not meet the MDGs and they are also not on track to meet the Sustainable Development Goals (SDGs).
Some of the contributing factors include the weak health leadership, systems and programs. In addition, less attention has been given to ascertain the quality of program design from the start and to assess the strength, effectiveness, efficiency, equity, scalability and sustainability of programs throughout implementation.
